Meredith Grey is an accomplished doctor and loyal friend. But the Grey’s Anatomy season 15 premiere subtly reminds us how she ties the whole show together. Here’s our recap.

When Grey’s Anatomy usually promises something, it delivers. In this case, the “season of love” begins with Meredith Grey’s wild desires — romantic fantasies of some of her co-workers, including Tom, Jackson, and, of course, DeLuca.

To explain these fantasies, she tells us about the “insula,” otherwise known as the pea-sized region in the brain where some emotions like pain, love and addiction live.

As Meredith stumbles through a sequence of fantasies and welcome distraction from her kids, we check in with newlyweds Alex and Jo on their honeymoon. Meanwhile, everyone recovers from their post-wedding hangovers on their way to the hospital.

Although Bailey is ready to jump back into work and prepare for the dawn of a new Grey Sloan, Teddy bursts her happy bubble with news that she won’t be accepting the position of interim chief. Bailey doesn’t understand the whole story, but once Teddy hears about Owen’s domestic life with Amelia, Betty and Leo, she chooses not to stay. On her way out, she runs into Owen and Amelia stepping off the elevator with Leo. The situation begins awkwardly, but Teddy finds a way to get out quickly.

Jackson, Maggie and DeLuca walk to work together, where DeLuca tries to reconcile kissing Meredith at the wedding. Jackson tells Maggie how happy he is for April, but mid-conversation, DeLuca spots a car speeding toward them and pulls them out of the way. Unfortunately, he’s unable to save the young woman on a bicycle who is struck and badly mangled by the crash.

Elsewhere, Webber and Roy receive a male patient with terrible stomach pain. Webber asks for Carina’s assistance, and as she walks down the hall with her brother, it’s easy to wonder Grey’s swapped her out for Arizona. While Carina is certainly charming, fans had a long history with the resident Good Man in the Storm, so it’s hard to say goodbye.

But then, Carina looks at an x-ray, and asks, “Are we taking bets on what this is?” as she checks the scans of a male patient who stuck what could be a candle too far up … you know, up there. Further, she fields questions from Roy and gracefully explains “straight men stupidity” and the prostate in her wonderful Italian accent.

To be fair, she’s can be quite the charismatic substitute.

The driver who lost consciousness behind the wheel, Cece, is attended to by Maggie until she’s pulled away to attend to Teddy. Cece appears to be okay, so she promises to stay by the cycler’s side all day. Well, at least, until the orthopedic surgeons arrive. The new “ortho god” is Chris Carmack’s Dr. Lincoln, who, despite Nisha’s drugged advances, sets a plan into motion to free her leg from her bike.

Upon finally meeting Meredith and Amelia’s younger sister, Teddy explains to Maggie how she is 11 weeks pregnant and may have a blood clot. She needs Maggie to prescribe her some medication so she can get on a plane and leave. Instead, Maggie thinks admitting Teddy would be a better idea and easily figures out that Owen may be the father of her baby, much to Teddy’s dismay.

While Maggie attends to Teddy, Meredith tends to Cece, who can read Meredith’s hunger like a book. After all, Cece is a matchmaker. To quell her desires, Meredith asks Bailey about the position of interim chief. Having been asked by both Amelia and Webber already, Bailey declines to discuss it any further with anyone. So, Meredith continues to treat Cece and explains that her heart medication is effectively poisoning her kidneys.

On their honeymoon, Jo’s mind remains on work while Alex envisions their life with 3 kids living in Boston and building snowcastles. She thinks that there’s a way to destroy cancer cells by introducing suicide cells into the body. Alex lends his support by making sure she eats and reviewing her notes.

Webber takes his patient into surgery, where the candle (that is actually a can of hairspray) inside the man explodes. Roy’s arm catches on fire, but DeLuca acts quickly and puts it out. Afterward, DeLuca and Webber cannot bring the man back, prompting Carina to help explain to the wife what happened.

Owen, Dr. Lincoln, a.k.a. Link, and Jackson head into surgery with Nisha. Taking Tom’s advice to fight for what she wants, Amelia decides to watch from the gallery while Owen is in the OR. Once they go home, Amelia shares that she has feelings for him, too. Unable to resist the call of domesticity, Owen reciprocates.

Unfortunately, the person who won’t be reciprocating is Maggie when Jackson implies he may want to get married soon.
